Lifeline Express, a medical operation train is an initiative driven by the Impact India Foundation to provide better health facilities to the masses of rural India. It is based on a novel concept of providing free and comprehensive rural medical facilities in the form of surgical intervention, rehabilitation and preventive treatment of curable disabilities including cardiac checkups, correction of polio, treatment of cleft lip and burn contracture.

The Express is a fully functional five-bogey train with operating theatre facilities, examination equipment and rest areas, and is parked in a designated area for a month. It enables poor and disabled patients to live in camps close to the railway station where the train is stationed and allows them to be examined and operated on by specialists.
